Use this system to produce a printed funding request which you must sign and send to KHC for approval. If you are returning to complete a request already started, enter the Request ID and password below and your existing request will be displayed for editing/printing.

To start a new request, click here. Once you begin your request and enter some basic information, the system will provide you a Request ID and password. You should make note of those in order to return to the request at a later time. (Note: the password assigned IS case sensitive).
